Next Steps: 

STEP 1. Schedule your LabCorp appointment 

• Select “Labs & Appointments” 
• Enter your Zip Code and select service “Employee Wellness with body measurement” 
• This will bring you to a list of nearby LabCorp locations. Select your preferred location and click “Make Appointment”. 
• Remember, you will be fasting for the 12 hours prior to your appointment, so keep this in mind when selecting a time.
• Choose your date & time for your appointment. 
• Enter your personal information. Please use your legal name. 
• Billing/Financial Details - Select “I have already paid or someone else is responsible”. 
• Enter your contact information. If you provided your email address, you will receive a confirmation email containing your confirmation number. You can use your confirmation number to reschedule or cancel your appointment if needed. 

STEP 2. Bring your voucher to your appointment 

• Remember to fast for 12 hours before your scheduled screen time. Please refrain from eating or drinking anything other than water. You may also drink decaf black coffee during your fast (avoid sweeteners and creamers). 
• Continue to drink water regularly, drink one 8 ounce glass one hour prior to your blood draw.
• Continue taking any medications as prescribed. 
• You must bring a photo ID to the screening 
• Please note: If you are diabetic or hypoglycemic, please follow your doctor’s instructions regarding fasting. 

• Do not smoke or chew tobacco for at least 1 hour prior to the exam 
• Limit your intake of salt and high cholesterol foods 24 hours prior to your exam 
• Do not engage in strenuous physical activity 24 hours before your exam 

STEP 3: What to expect after your screening

You will receive your results in 2-4 weeks, your results will be available in the portal. You will receive an email with instructions, see your deductible rewards, and much more.
